IF you are under the age of 18, you must have a parent or legal guardian bring you to your appointme
- Valid photo ID for patient, required by the state of Virginia. This can be a State Issued ID, a military ID, a passport, or a student ID for the current year. A print out of the current Student Profile from their school may also be used.
- Valid photo ID for parent or legal guardian
- Parents must bring birth certificate; legal guardian must be guardianship paperwork.
- Notarized parental consent. This is REQUIRED by the State of Virginia.
- Everything must be printed out, cannot take documentation on a cell phone, tablet or laptop.
- Only patient and parent/guardian are allowed in office.
