Abortion patients under 18
If you are under the age of 18, you must bring a parent or legal guardian to your appointment. Here
- Valid photo ID for patient. Can be a state issued ID card, military ID, or passport. May also use a printed out student profile page from the school (must include photo of student). ID's cannot be voided or expired.
- Valid photo ID for parent or legal guardian. ID's cannot be voided or expired.
- Parents must bring birth certificate; legal guardians must bring guardianship paperwork from the court.
- Everything must be printed out and brought to the appointment. We cannot accept any ID's or documentation on a cell phone, tablet or laptop.
- We ask that only patient and parent/guardian come to appointment.
- NOTARIZED parent consent. This is required by Virginia State Law. Without this notarized documentation, patient cannot be seen. This can either be picked up in office before appointment, emailed or faxed. Please call office at (757) 461-0011 if you choose to have documentation sent to you.
